Mid-Year Test (Grade 1-2)

Ocean Blow Hole

This is a test where we are going to get to show how much we have learned this year in health.  For some of the questions you may have to draw pictures, for others you may have to copy the sentences and fill in the words.

1) Draw a picture of what you should do each time before you eat food.  This will help to get rid of the germs on your hands.

2) Copy this sentence:

I should ________ my teeth 3 times a day and take a _______ every day.  This helps to get rid of germs and keeps me clean.

3) Copy this sentence:

I should never throw_________ at my friends face in the winter because they may get hit by ice or a rock.  Also I should never ever walk on ____________.

4) Draw the phone number you should call if your friend falls through the ice or if you need help.  You should never call this number unless it is an emergency.

5) How many meals a day should you eat? Also draw a picture of a healthy drink you could drink with each meal?

6) Tell me what you should do if someone is bullying you in school or at recess.

7) Why is it important to wear warm clothing in the winter?

8) Sit with a friend and draw a picture of three healthy things you can do over vacation.

9) Go to scrubclub.org and see if you can "kill" the germs!
